Job Title: Per Diem PTA - Weekend Shifts Only

Company: Home Health Care, Inc.

Location: Twin Cities and Southwest MN

Compensation: $42-$46 per visit

• Serving clients one-on-one in their homes

What You’ll Do:

• Carry out PT plans and monitor client progression
  
• Maintain EMR documentation
  
• Act as a liaison between clients and supervising PTs
  

Requirements:

• MN-licensed PTA
  
• Availability Saturdays and/or Sundays
  
• Strong time management and reliability
