Role Description

This is a full-time on-site role for a Medical Social Worker at Mountainview Home Health in Yakima, WA. The Medical Social Worker will be responsible for medical social work, discharge planning, case management, and social services.

Qualifications

• Medical Social Work and Social Work skills
• Experience in Discharge Planning and Case Management
• Knowledge of Social Services
• Excellent interpersonal and communication skills
• Ability to work in a fast-paced healthcare environment
• Must have a Bachelor's or Master's degree in Social Work
• Previous experience in a healthcare setting is a plus

Must have agency affiliated license or LICSW to practice in the home health setting.
