Position Summary

The Director of Nursing (DON) oversees the clinical operations of Home Healthy, ensuring high-quality, compliant care delivery in alignment with CMS, AHCA, and HIPAA regulations. This full-time or part-time role is ideal for an experienced RN leader passionate about home health, with a focus on staff supervision, quality improvement, and leveraging technology to drive patient outcomes. The DON will collaborate with referral sources (e.g., hospitals, physicians) to support seamless care transitions and agency growth.

Key Responsibilities

Clinical Leadership

  • Oversee delivery of skilled nursing, therapy, and aide services, ensuring adherence to care plans and medical necessity standards.
  • Supervise clinical staff (RNs, LPNs, aides), including hiring, training, and performance evaluations.
  • Implement 24/7 monitoring technology to enhance care delivery, reduce readmissions (<15% target), and improve patient satisfaction (85%+ HHCAHPS goal).

Compliance & Quality Assurance

  • Ensure compliance with CMS Home Health Quality Reporting Program (HH QRP), AHCA regulations (Fla. Stat. § 400.474), and HIPAA.
  • Conduct quarterly audits of OASIS assessments, claims, and patient records to maintain Care Compare benchmarks.
  • Develop and update policies/procedures to align with federal and Florida regulations.

Staff Development & Training

  • Lead mandatory training on compliance, and clinical best practices (e.g., wound care, chronic disease management).
  • Foster a positive, collaborative environment, addressing issues like workplace toxicity promptly.
  • Mentor staff to achieve certifications (e.g., Certified Home Health Administrator).

Referral & Partnership Support

  • Collaborate with hospitals and physicians to streamline referrals, emphasizing proactive monitoring (15–20% readmission reduction).
  • Ensure timely initiation of care (95%+ within 48 hours) and coordinate with intake staff for seamless transitions.
  • Represent Home Healthy at community events to build referral networks.

Performance Improvement

  • Monitor key metrics (e.g., acute care hospitalization rates, patient outcomes) and implement quality improvement plans.
  • Analyze Sensi.ai data to identify care anomalies and optimize caregiver-client pairings.
  • Report quarterly performance to leadership, aligning with hospital partnership goals.

Qualifications

  • Education : Bachelor’s degree in Nursing (BSN) required; Master’s degree (MSN) or healthcare administration preferred.
  • Licensure : Active Florida RN license in good standing; AHCA-required background screening (Level 2).
  • Experience : Minimum 5 years of nursing experience, with 2+ years in home health leadership; familiarity with CMS regulations and OASIS assessments.
  • Skills :
  • Strong leadership and team management (10–20 staff).
  • Proficiency in EHR systems. Knowledge of KanTime is a plus.
  • Excellent communication for referral source engagement.
  • Knowledge of Medicare/Medicaid billing and compliance.
  • Availability : Flexible 12–40 hours/week, including some remote work and on-call duties.
  • Preferred : bilingual (English/Spanish) a plus for Miami-Dade/Broward.
  • Location: Hialeah or Broward with some commute between the two offices.
